Protocol Template

“Tumor” in the table below refers to:

  • The PTV, if no boost object is available
  • The PTV – boost, if a boost object is available

Prescription

Fractions

Enter the number of fractions for treatment.

Prescription/Coverage Dose (Gy)

Enter the prescription/coverage dose. This value must be between 1.0 – 99.9 Gy.

Desired Coverage Volume (%)

Enter the percentage of a tumor volume that should ideally be covered by dose during treatment. This value must be between 50 – 99.9%.

Tolerated Coverage Volume (%)

Enter the minimally tolerated percentage of a tumor volume that should be covered by the dose during treatment. This value must be between 50 – 99.9%.

Maximum/Desired Dose Constraint Dose (Gy)

Enter the absolute maximum/desired dose constraint or leave empty. This value must be between 1.0 – 99.9 Gy.

If the maximum/desired dose constraint is set, the prescription/coverage dose of the tumor must be 50 – 100 % of the maximum/desired dose constraint dose to ensure that the % IDL is between 50 – 100%, otherwise the prescription/coverage Dose will be adapted accordingly.

This value must be ≤ the maximum/desired dose constraint dose of the boost object (see Boost below).

  • If SRS Prescription (controlled inhomogeneity) = false: This setting is optional and set as the maximum dose constraint.
  • If SRS Prescription (controlled inhomogeneity) = true: This setting is required and set as the desired dose constraint.

Maximum/Desired Dose Constraint Volume (%)

Enter the maximum/desired dose constraint volume. This value must be between 0 – 50%.

  • If SRS Prescription (controlled inhomogeneity) = false: This setting is set as the maximum dose constraint.
  • If SRS Prescription (controlled inhomogeneity) = true: This setting is set as the desired dose constraint.

Tolerated Dose Deviation of Maximum/Desired Dose Constraint (%)

Enter the tolerated deviation from the maximum (for non-SRS prescription) or desired (for SRS prescription) dose constraints. This value affects the colored symbols in the plan analysis table. This value must be between 0 – 10%.

SRS Prescription (controlled inhomogeneity)

True: Enable SRS Prescription (controlled inhomogeneity) mode. The algorithm attempts to distribute the dose inhomogeneously within the tumor volume. The desired dose constraint is treated as a prescription point to be reached.

False: Disable SRS Prescription (controlled inhomogeneity) mode. The algorithm attempts to distribute the dose homogeneously within the tumor volume. The algorithm attempts to not exceed the maximum dose constraint.

Boost

Use for Normalization

True: The algorithm uses the boost object for normalization.

False: The algorithm uses the PTV – boost for normalization.

Prescription/Coverage Dose (Gy)Enter the total dose to be applied to the boost.

For example, if the PTV – boost should receive 15 Gy and the boost an additional 3 Gy, then the total dose is 18 Gy. This value must be between 1 – 99.9 Gy.

Desired Coverage Volume (%)

Enter the percentage of a boost volume that should ideally be covered by dose during treatment. This value must be between 50 – 99.9%.

Tolerated Coverage Volume (%)Enter the minimally tolerated percentage of a boost volume that should be covered by the dose during treatment. This value must be between 50 – 99.9%.

Maximum/Desired Dose Constraint Dose (Gy)Enter the maximum/desired dose constraint dose to be applied to the boost or leave empty. This value must be between 1 – 99.9 Gy.

If the maximum/desired dose constraint is set for the boost object, the prescription/coverage dose of the boost object must be 50 – 100 % of the maximum/desired dose constraint dose to ensure that the %  IDL is between 50 – 100% otherwise the prescription/coverage dose will be adapted accordingly.

  • If SRS Prescription (controlled inhomogeneity) = false: This setting is optional and set as the maximum dose constraint.
  • If SRS Prescription (controlled inhomogeneity) = true: This setting is required and set as the desired dose constraint.

False: Disable SRS Prescription (controlled inhomogeneity) mode. The algorithm attempts to distribute the dose homogeneously within the boost volume. The algorithm attempts to not exceed the maximum dose constraint.

Most Important OAR

Enter the organ type of the most important OAR. The name must match an item in the OAR Constraints (Value List).

OAR Constraints (Value List)

Create a list of OARs and their constraint information.

Define for each OAR:

  • The organ Type that matches the Type in Object Management

  • A tolerated dose deviation
  • A maximum dose constraint

  • Up to four dose constraint points

Dose Display

Highlight Dose (Gy)

Enter the default value in Gy for the adjustable highlight field in the Dose Display function. This value must be between 1.0 – 99.9 Gy.

Show Half Prescription

Choose to enable or disable the half prescription value in the Dose Display function.

True: Enable the half prescription value.

False: Disable the half prescription value (default).

Show Maximum Limit

Choose to enable or disable the maximum dose limit.

True: Enable the maximum dose limit.

False: Disable the maximum dose limit.

Maximum Limit (%)

Enter the maximum dose limit relative to the prescription/coverage dose (%) to display in the Dose Display function within the applications. This value must be between 100 – 200%.

Show Absolute Dose

Choose to display absolute or relative dose values in the dose legend as the default. You can change this setting in the software by double-clicking on the dose legend.

True: Display the absolute dose.

False: Display the relative dose.

Dose Lines (Value List)

Dose Line 1…

Dose Display function according to your needs.

Maximum: 3 values.

Dose (Gy)

Enter the dose (Gy) for each dose line. This value must be between 1.0 – 99.9 Gy.

Dose Grid Parameters

Dose Grid Resolution (mm)

Enter the grid resolution. This value must be between 1.0 – 4.0 mm.

Monte Carlo Spatial Resolution (mm)

Enter the Monte Carlo spatial resolution. This value must be between 1.0 – 4.0 mm.

Monte Carlo Statistical Uncertainty during VMAT optimization (%)

Enter the Monte Carlo statistical uncertainty during VMAT optimization. This value must be between 1.0 – 8.0%.

Monte Carlo Statistical Uncertainty for final forward dose calculation (%)

Enter the Monte Carlo statistical uncertainty for final forward dose calculation. This value must be between 1.0 – 8.0%.

Monte Carlo Dose Result TypeThere are two Monte Carlo dose result types:

  • Dose to water
  • Dose to medium
